The Rhode Island Historical Preservation & Heritage Commission (RIHPHC) has a plaque program to recognize properties listed in the State Register. If you own a property listed on the State or National Register of Historic Places, you may purchase a plaque to display.

Prior to ordering a plaque, property owners must confirm with RIHPHC that the property is listed in the State Register. Visit our State Register page to consult the working list of registered properties. Then fill out the Rhode Island State Register Request form below and submit a copy to RIHPHC via email

If your property is listed, RIHPHC will email you the signed certificate request form and an order form. Include a copy of the certificate request form along with your order form to purchase a plaque directly from the manufacturer, Healy Plaques in Manville, Rhode Island. 

Each 4.25" x 7.25" plaque is made of cast aluminum with a black painted background, finished with a protective polyurethane coating. The plaque is pre-drilled for easy installation and comes with 2 painted wood screws. As of January 1, 2024, the total cost per plaque is $207.57 ($174.00 + $13.58 RI Sales Tax + $19.99 Shipping).
